Full Job Description :- 

Summary: As a Home Health care nurse, you are required to provide one-on-one care for patients in their homes. It involves assisting patients in their Activities of Daily Living such as bathing & dressing as well as more specialized services like wound care, medication management, and IV therapy.

Duties & Responsibilities:

Before accepting a patient for home healthcare an effective and comprehensive Multidisciplinary Patient Assessment Shall Be Conducted.
That includes but not limited to medical history, physical, social and psychological assessment of patient’s needs.

Risk Assessment must be conducted and documented upon admission. Healthcare professionals should use a formal risk assessment process to assess skin integrity, Nutritional Assessment as well as risk of falls for such patients.
Care shall be delivered by licensed individuals and competent multidisciplinary team. Also, it should be based on the best available evidence and within the scope of practice.
Initiation and evaluation of a Patient Care Plan
Observation and assessment of patient’s condition, vital signs assessment.
Administration of medications and instruction on Medication such as (Injections, IV’s, inhalation, oral, infusions, rectal, and enterals) by a Registered Nurse only.
Management of palliative care patients who have a diagnosis of cancer or advanced progressive illness/terminal life-threatening disease of disabilities including pain management

Daily insulin injections, supervision of patients with diabetes and monitoring blood glucose
Tube feeding Management
Nasopharyngeal and tracheostomy aspiration
Indwelling catheter care and maintenance
Wound care
Ostomy care and maintenance
Assessment for oxygen therapy requirements
Point of care testing such as Blood Glucose Level, Disptick Urine
Naso-gastric tube care and maintenance
Foley Catheter Insertion
Pain management
Continence assessment and management
Education and counselling, consistent with the type/nature of treatment and/or care management needs.
Minimum Qualification & Experience:

Nursing graduate with license Dubai health Authority
Evidence of team working skills with ability to work using own initiative.
BLS certification or working towards.
Ability to fulfil criteria outlined in the job description.
Knowledge / Technical Skills:

Nursing graduate with license from their home country
Evidence of team working skills with ability to work using own initiative.
